Cuộc sống yên bình, dễ chịu của Zhivago ở Moskva chìm trong hỗn độn khi xảy ra Thế chiến I và cuộc Cách mạng Nga. Zhivago chia tay vợ con tòng quân làm bác sĩ quân y.

Trong khi đó nhân vật Lara tình nguyện làm y tá với hy vọng tìm kiếm tung tích chồng cô. Chiến tranh bùng nổ khắp nơi, Zhivago và Lara được bổ nhiệm công tác chung tại một nhà thương ở tỉnh nhỏ xa xôi. Zhivago bị thôi thúc bởi những cảm tình cho Lara…

 

Bác sĩ Zhivago là câu chuyện không nhuốm màu thời gian, vượt qua kỷ nguyên bối cảnh trong phim và cả thời kỳ bộ phim được phát hành. Trong khi đó, các diễn viên Omar Sharif (diễn viên Thổ Nhĩ Kỳ) và Julie Christie (Anh) như thể sinh ra là để hóa thân thành Zhivago và Lara, mặc dù họ chẳng “dính dáng” gì tới nước Nga.

Phim được dàn dựng theo cuốn truyện cùng tên của nhà văn Nga Boris Pasternak và sự ra đời của cuốn tiểu thuyết này cũng kịch tính chẳng kém gì câu chuyện của nó.

Đây là tiểu thuyết duy nhất của Pasternak, được ông hoàn tất vào khoảng năm 1956, song không được xuất bản ở Liên Xô khi ấy vì có các nội dung nhạy cảm. Năm 1957, Pasternak được nhà báo Italy Sergio d’Angelo đề nghị gặp gỡ khi ông tới Moskva.

Lúc đôi bên gặp nhau, Pasternak đã trao cho d’Angelo bản thảo chưa xuất bản của cuốn truyện và nhà báo này đã đưa bản thảo ra khỏi Liên Xô. Sau khi mang về Italy, D’Angelo đã liên hệ để cuốn truyện được xuất bản. Nó nhanh chóng trở thành tác phẩm ăn khách quốc tế, nhận được nhiều lời ca ngợi.

Năm 1958, Pasternak đã được trao giải Nobel Văn học. Pasternak qua đời hồi năm 1960, 5 năm trước khi tiểu thuyết của ông được đưa lên màn bạc.
